ADMISSION PROCEDURE

8. DOCUMENTATION

Following documents must be attached with the application form:

         1) Matric Certificate / ‘O’ Level Certificate.

         2) Matric mark sheet/‘O’ Level Equivalence Certificate.

         3) Intermediate Provisional Certificate / A Level equivalance certificate.

         4) Intermediate Science Marks Sheet / A Level grade statement certificate.

         5) Candidate’s Domicile Certificate of district Karachi.

      6) Candidate’s Permanent Resident Certificate (PRC, on form – C) district
  Karachi.

      7) Candidate’s NADRA National Identity Card or ‘B’ form, (if he/she is
           below the age of eighteen years) issued from Karachi.

         8)  Domicile Certificate of father of Sindh Province on form P-1 and National Identity Card  from NADRA of Sindh Province.

        Note :

§       Any candidate found using Domicile / PRC of more than one district / place in Pakistan will be disqualified for admission in any medical institution in Sindh Province, and the case will be referred to the concerned authorities for investigation.

§          The domicile certificate of mother (Sindh Province) shall only be accepted in case where father has expired and no domicile certificate was issued to him or there has been remarriage / separation for which a valid document from the judge of the high court has been produced.

          9)  The candidates applying in overseas category should should submit:

(a) Relevant documents including A Level / equivalent certificate attested and forwarded by the head of the institute and respective Pakistan embassy.

         (b) Employment certificate of father attested and forwarded by respective Pakistan embassy/consulate.

         10)  Hafiz-e-Quran Certificate, if applicable.

         11)  Disability certificate if applicable.

         12)  Six recent passport size photographs with the name, 3 attested from front & 3 attested, from back

         13)  Affidavit by the candidate on the stamp paper of Rs. 20/- affirming that he /she will abide by the rules and regulations of admission and endsorsed by father / guardians        .

This is to certify that we have read the rules and regulations mentioned in this application form and the prospectus and agree to abide by them. we also undertake that we will not breach the normal process of admission and disturb the administration discipline.
