1. Pay order in favour of Dow University of Health Sciences for specific category:

   1. Merit No. 1 till 426 should submit pay order of Rs. 24,150/- (for general merit

   2. Merit No. 427 and onwards can bring pay order of Rs. 250,000/- (for UEDP-MBBS, Self Finance

   3. CANDIDATES interested for BDS can bring two pay orders (Rs. 60,000/- for general merit), (Rs. 300,000/- for UEDP local

   Similarly candidates for MBBS whose merit is on borderline can bring 2 pay orders, one for general merit and the other for UEDP local. The extra pay order will be stamped and signed there and then by the Registrar DUHS for cancellation.

2. All original documents (photocopy of which has already been submitted with the application form), for submission.

3. Candidates of all categories must bring Affidavit on Stamp paper of Rs. 80/- as per following specimen:
