Application Submission Dates

by hand at University of Health Sciences (UHS) Lahore from 9th to 11th October, 2010(including Sunday) from 08:30 AM to 02:30 PM, with one set of attested copies of following documents:

  1. Matric (SSC or equivalent) certificate

  2. FSc (pre-medical) or equivalent certificate.

  3. Domicile certificate of the candidate.

  4. Three passport size coloured photographs. (One to be pasted on the application form and to be attested on the front andother two, attested on the back, to be attached with the application form).

  5. Two self addressed envelops with Rs.30/- postage stamps each.
